Stage 1: Application Approval

As you embark on this journey with your child, remember, every brave step forward marks a path filled with hope and resilience. You’re not just applying for a hairpiece; you’re opening a new chapter of joy and self-confidence for your child.

Find a Local Certified Service Provider

It’s time to find and contact a local stylist, complete the online pediatric wig program application, and mail the required documentation and photographs.

Using our link below, please search for a Certified Service Provider (specifically trained by Wigs For Kids) that is closest to you.

What if I can’t find a local provider?

We recommend using search engines, such as Google, to look for a:

  • Hair replacement center; or
  • Professional salon that has experience with hairpieces; or
  • Salon that is willing to complete our online training (free of charge) before they assist with the measurements and fitting for the hairpiece.

Contact the Certified Service Provider

Introduce yourself as a Wigs For Kids applicant, and confirm their willingness to assist with measurements and fitting/styling of your child’s hairpiece once your application is approved. You will need the following information:

  • Salon/center full name
  • Stylist full name
  • Salon/center full address and phone number
  • Best email address for reaching the stylist at the salon/center

Note: Wigs For Kids will provide the stylist with the tools necessary to properly fit and help your child maintain their hairpiece. Once your application is approved, we will ship the necessary Measurement Packet to you.

Complete and Submit the Online Application

Once you’ve identified, contacted and confirmed your stylist, please complete and submit the online application.

Note: Only a child’s legal guardian can submit the online application form for the program.

Mail Documentation and Photographs to Wigs For Kids

Gather supporting documents for the application:

  1. The Wigs For Kids Medical Information Form signed by your medical professional
  2. Prescription for ‘cranial prosthesis’ OR a referral letter stating diagnosis from your medical practitioner
  3. Legal age document (copy of one of the following are acceptable: birth certificate, passport, state ID)
  4. Current color picture of child
  5. Color picture of child prior to hair loss or picture of a hairstyle they’d ideally like to wear

Once you’ve gathered these documents, please mail them to:

Note: We can’t consider your application until we receive these documents from you. Please make every effort to provide the documentation right away. We strive to provide medical hair loss wig assistance for kids like yours as soon as possible.

Once you’ve completed the following steps, please allow 2–3 weeks for your documents to arrive at Wigs For Kids and for our team to review and process your application. We will be in touch with you soon!

  1. Confirmed your stylist (Certified Service Provider)
  2. Submitted your online application
  3. Mailed your documentation and photos
